The downloadable questionnaire below should only take a few minutes to fill in – we are trying to get a ‘snap-shot’ of current practice rather than carry out an in-depth survey. Also, this is not a test! Please don't feel you SHOULD be doing any of these – or hesitate to return the form if you are not doing any (and we certainly won’t personalise any of the information you send us).
